APPENDIX 3
The Questionnaire

Certain minor alterations and additions were made to the questionnaire.

Instead of “the last 24 hours”, “yesterday” was used in all questions about meals and foods.

The following additions were made:

i. “Is your mother working?” The analysis of this question did not yield useful results.
ii. “Is milk available at your place of work? Do you take it?”
iii. Other Vegetables and Fish were added to the list of foods.
